Live Healthy Lane is a formal partnership of key partners. Together, Lane County Community Advisory Council, Kaiser Permanente, Lane Community Health Council, Lane County, Pacific Source Health Plans, and Trillium Community Health Plan oversee the periodic assessment of health issues and needs in Lane County (The Community Health Assessment; CHA) and the development and implementation of the Community Health Improvement Plan (CHP) in collaboration with the community. Reducing health disparities, promoting health equity and improving overall population health is the central purpose of this work.

Live Healthy Lane involves everyone. Only through a collective effort — in which all of us work together toward a shared purpose — can meaningful change be achieved in a region as large and diverse as Lane County. Together we can achieve more than each of us can alone.

Rural Coalition Partners

The Siuslaw Vision and Be Your Best - Cottage Grove are two rural community partners of Live Healthy Lane, working together to improve the health and well-being of those in Lane County. These coalitions actively engage rural communities in Live Healthy Lane's efforts including the Community Health Assessment (CHA) and Community Health Improvement Plan (CHP.)
