Community Health Assessment
The Lane County Regional Community Health Needs Assessment (CHA) is more than charts and graphs. It is more than statistics and numbers. This assessment introduces a new approach to community health, one where decisions about programs and interventions are not based solely on “the numbers,” but also on what community members feel is important as they strive to live a healthier life. Using the voice of the community and supporting data, the findings from the assessment are used to develop a shared Community Health Improvement Plan (CHP) which identifies how we'll work together to address some of the important issues.

The 2018-19 CHA will inform Lane County's 2020-24 Community Health Plan (CHP), formerly known as the Community Health Improvement Plan (CHIP).
Care Integration Assessment (CIA) - Pilot
- What: explores potential opportunities and threats to system/service integration, as required by House Bill 2675
- Who: Oregon Health Authority (OHA), Trillium Community Advisory Council (CAC), Live Healthy Lane partners, and county-wide agency members
Forces of Change Assessment (FOCA)
- What: explores positive and negative forces predicted to influence health and health systems in the next five years
- Who: systems-level representatives in Lane County
Local Public Health Systems Assessment (LPHSA)
- What: explores organizational contributions to public health, interconnectedness of activities, and how the public health system can be strengthened
- Who: systems-level representatives in Lane County
Community Themes & Strengths Assessment (CTSA)
- What: explores quality of life and well-being of the community by directly engaging community. The 2018-19 assessment asked: 1) Do the 2015 health issues continue to be a priority? 2) Do these priorities resonate with underrepresented populations?

- includes information about health behaviors like rates of tobacco use; births and birth outcomes; behavioral health statistics; infectious and chronic disease rates; and causes of death in Lane County
2015-2016 Process
To guide the process, we chose to use the Mobilizing for Action through Planning and Partnerships (MAPP) model because of its strong emphasis on community input. MAPP helps communities apply strategic thinking to prioritize public health issues and identify resources to address them. This community health improvement process has only been possible because of the amazing participation from hundreds of local organizations and thousands of community members. Thank you!
Step 1: Visioning
In June 2015, a multi-site community health visioning session provided the platform for 135 community members to share their views on the desired health of the region and what guiding values need to be in place to achieve that vision.
Step 2: Four Assessments
Between May 2015 and January 2016, four assessments were conducted with the help of thousands of community members and hundreds of organizations. These assessments provide a comprehensive snapshot of the specific health needs and opportunities in the Lane County region.
- Forces of Change Assessment: identified the trends, factors, and events that were likely to influence community health and quality of life, or impact the work of the local public health system.
- Local Public Health System Assessment: evaluated the components, activities, competencies, and capacities of our local public health system and how well the essential public health services are being provided to the region.
- Community Health Status Assessment: provided data on the community’s health and answered questions such as: How healthy are the people who live in the community? What does the health status of our community look like?
- Community Strengths and Themes Assessment: gathered the thoughts, opinions, and perceptions of community members in order to understand which issues are important to the community.
Step 3: Prioritize Issues
What needs to be addressed in order for our community to achieve our vision? During Live Healthy Lane: Defining Our Future - a multi-site community event in February 2016, over 250 community members voted on the prioritized issues to be addressed through the 2016-2019 Community Health Improvement Plan (CHIP):
- Promote access to economic and social opportunities necessary to live a healthy life.
- Promote healthy behaviors and engage the community in healthy living.
Next Steps
Live Healthy Lane continues to work with partners, stakeholders, and community members to implement and evaluate the 2016-19 Lane County Regional CHP.
The 2018-19 CHA will help inform 2020-24 CHP strategies.
