Lane County's Community Health Assessment (CHA) and Community Health Improvement Plan (CHP) reports and additional resources are available for the community to use to inform local health efforts.
Introducing the CHA & CHP
The CHA uses a strategic framework for prioritizing community health issues and developing strategies to improve health outcomes. CHA results are used to develop a CHP, which outlines a five-year strategic effort for addressing priority health issues in Lane County's communities.

- increase economic and social opportunities,
- increase health behaviors to improve health and well-being, and
- strengthen cross-sector collaborations.
This work is implemented in collaboration with Lane Community Health Council, Lane County Public Health, PeaceHealth, Trillium Community Health Plan, United Way of Lane County, Live Healthy Lane partners, and YOU to help improve health for all who live, learn, work, and play in Lane County. Together we can do more than each of us can do alone.

The 2018-19 CHA will inform Lane County's 2020-24 Community Health Improvement Plan (CHP), formerly known as (CHIP).
Care Integration Assessment (CIA) - Pilot
- What: explores potential opportunities and threats to system/service integration, as required by House Bill 2675
- Who: Oregon Health Authority (OHA), Trillium Community Advisory Council (CAC), Live Healthy Lane partners, and county-wide agency members
Forces of Change Assessment (FOCA)
- What: explores positive and negative forces predicted to influence health and health systems in the next five years
- Who: systems-level representatives in Lane County
Local Public Health Systems Assessment (LPHSA)
- What: explores organizational contributions to public health, interconnectedness of activities, and how the public health system can be strengthened
- Who: systems-level representatives in Lane County
Community Themes & Strengths Assessment (CTSA)
- What: explores quality of life and well-being of the community by directly engaging community. The 2018-19 assessment asked: 1) Do the 2015 health issues continue to be a priority? 2) Do these priorities resonate with underrepresented populations?
