Community Health Assessment Design Team (CHADT)

​The 2024 Community Health Assessment was led by the Community Health Assessment Design Team. Their first task was to update Live Healthy Lane’s vision for a healthy and fair community. In June 2024, they hosted community conversations across Lane County. Based on those discussions, the team created a vision statement to guide the 2024 Community Health Assessment.

Focus group and survey data about organizations and partners help us understand what we can do together to improve health and wellness in our community.

​​​​Community Context Assessments
Focus groups and a survey for community members which was available in Spanish and Chinese, explores the strengths, resources, environment, personal experiences and forces of change that affect health in Lane County. Three assessments were conducted.
Community assets and barriers - explores our community resources, challenges to being healthy, and what makes Lane County unique.

Built environment - explores the physical spaces and resources in Lane County that affect health, like housing, transportation, and schools. We looked at how these resources are different across communities. Exciting news coming soon!

Forces of change - examines outside influences that may affect health in Lane County and which communities are impacted the most.

What's next? Stay informed!

A group of community and organizational leaders have come together to form the Community Health Improvement Coalition (CHIC). This group will prioritize the CHA issues to identify the CHP priorities and develop promising strategies to be written into the CHP. Are you interested in staying updated on the progress of the new 2026-2030 CHP? The CHIC will be announcing short term engagement opportunities soon.
