Community Health Assessment Design Team (CHADT)
The 2024 Community Health Assessment was led by the Community Health Assessment Design Team. Their first task was to update Live Healthy Lane’s vision for a healthy and fair community. In June 2024, they hosted community conversations across Lane County. Based on those discussions, the team created a vision statement to guide the 2024 Community Health Assessment.
