Community Health Improvement Coalition (CHIC)
The Community Health Improvement Coalition (CHIC) brings together a diverse group of community leaders and organizational representatives who guide the development of strategies that address Lane County’s health priorities. Members include both individuals serving in a personal capacity and representatives of local organizations, agencies, and sectors. Together, they ensure that the CHP reflects the voices, experiences, and expertise of our community.

The Siuslaw Vision and Be Your Best - Cottage Grove are two longtime rural community partners of Live Healthy Lane, working together to improve the health and well-being of those in Lane County. These coalitions actively engage rural communities in Live Healthy Lane's efforts including the Community Health Assessment (CHA) and Community Health Improvement Plan (CHP.)
