Our Work
Health equity isn't achieved through one project. It requires consistent collaboration, long-term commitment, and shared vision. Our work reflects the collective effort of partners, community members, and local organizations working together to remove barriers and build healthier communities.

Current Work
Transportation is one of the highest-impact social drivers of health in our region. Limited or unreliable transportation affects access to medical care, employment, food, and community resources. Our current work focuses on:
- Identifying transportation gaps, focusing on rural areas.
- Working with community partners and local agencies to expand options.
- Supporting solutions that make essential services easier to reach.
Past Work
This project brought together community members, local leaders, and partner organizations to better understand the unique challenges rural residents face. Through listening sessions, data collection, and collaborative planning, the Rural Health Equity Project helped set the foundation for today's CHP priorities.
Over the years, LHL has hosted educational events, convenings, and community workshops that:
- Elevated voices from across our region.
- Built shared understanding about health equity.
- Strengthened partnerships across sectors.
CHAs and CHPs shape our direction by identifying community needs and establishing clear strategies for action. Click on the button above to learn more about:
